AFLAC’s Personal Lifestyle Protector Cancer Plan
This plan includes comprehensive benefits for your family in the event of diagnosis of cancer. The plan also pays you a “wellness benefit” when you receive a cancer screening.

• One Parent Family...$20.72
• Two Parent Family...$28.89
AFLAC’s Personal Disability Income Protector
This plan provides a source of income if you are hurt due to an accident or sickness. This will assist in meeting your financial obligations without a paycheck. For rates, please speak to the TBTA AFLAC representative at the number below as rates will vary.
These premiums can be deducted bi-weekly via payroll deduction. Also the products are portable and can be carried with you at retirement at the same group rate. The rates above are BI-WEEKLY RATES.
